1. Download the "Examination Acceptance Form" from the link below and make sure to obtain the signature and approval stamp from the representative of your affiliated team (such as the coach).
※If you are not currently affiliated with a team, the representative's signature and consent stamp are not required.
Please use the U-18 Selection Examination Consent Form below.
2. Paste the "Participation Application Ticket" purchased in [STEP2] onto the examination acceptance form.
