※If you are participating in an event for elementary school students or younger, please provide the information of the guardian in the participant application. (1) Child's name (in hiragana), (2) Whether the child is enrolled in FC Tokyo School (if enrolled, please also provide the venue), (3) Affiliated team, (4) Grade. Please include this information in the "Contact Information for the Facility" section (Please provide all information only for the first time, and from the second time onwards, please only provide the child's name).

※"Contact Service When Available" is a system where an email is sent to all registered users when there are vacancies. The contact will be sent to everyone at the same time, and it will be on a first-come, first-served basis. Once the contact is sent, if you do not register again, you will not receive a notification when the next vacancy becomes available.

□Payment Information
Please make the payment at the FC Tokyo Park Fuchu front desk before the start of the event.

□Regarding the decision to hold the event
In case of rain or severe weather, the event may be canceled.
In that case, the decision will be made one hour before the start of the event. In the event of cancellation, we will inform you on the FC Tokyo Park Fuchu website and Twitter. Please note that we will not contact you regarding the cancellation.

□ Notes on facility usage
- Please do not bring your own ball (the facility will provide the ball).

The facility is on the rooftop of the building. The use of the ball outside of the pitch is not allowed.
・You cannot enter the pitch with spikes. Please come in training shoes or futsal shoes.
・There may be times when it is difficult to reach us by phone. Please understand in advance.
・Photos may be taken during events. They may be used by FC Tokyo or FC Tokyo Park Fuchu. Please understand in advance.
